OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency’s website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health home page (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Clark County
In April 2022 the Nursing Assistant Program suspended the certified nursing assistant license of Akberet M. Tekle (NC10083574). The Department of Social and Health Services determined that he neglected a vulnerable adult and placed Tekle on a registry that prevents him from being employed to care for vulnerable adults.
In April 2022 the Nursing Commission suspended the licensed practical nurse credential of Janet Elizabeth Scurlock (LP60178939) and denied Scurlock’s application for reinstatement of the license. The Oregon State Board of Nursing had revoked Scurlock’s license for failing to comply with substance use treatment and monitoring requirements.
Grays Harbor County
In April 2022 the secretary of health suspended for at least 36 months the mental health license of Judith L. Miniken (LH00010642). Miniken violated professional ethics rules when she ended a therapeutic relationship with a client who moved into her home.
King County
In April 2022 the Substance Use Disorder Professional Program and chemical dependency professional trainee (now substance use disorder professional trainee) Steven Lee Campbell (CO60733606) entered an agreement that suspends his license. Campbell didn’t enter a substance use program, which was required as part of a 2021 agreement.
Pierce County
In March 2022 the secretary of health suspended the agency affiliated counselor and registered nursing assistant licenses of Paul Thomas Boros (CG60249751, NA00133095). Boros engaged in physical contact with a patient at Western State Hospital after the patient blocked his path, attempted to steal his lunchbox, and tore up his medical paperwork.
Snohomish County
In March 2022 the secretary of health revoked the registered nursing assistant license of Kelvin Muthomi Njeru (NA60924253). Njeru was charged with rape after the assisted living facility where he worked reported the sexual assault of a patient and a DNA test showed a strong link to Njeru. He was suspended from practicing in November.
Thurston County
In April 2022 the secretary of health suspended the home care aide credential of Carolyn L. Shows (HM60774986). Shows financially exploited a patient she cared for by accepting a loan for $700 and the gift of a car valued at $2,200.
In March 2022 the secretary of health suspended for at least five years the registered agency affiliated counselor license of Jeremy Samuel Kochrian (CG60148814). While providing family therapy to a client and her son, Kochrian exchanged sexually explicit text messages with the client and had sex with her multiple times.
Yakima County
In March 2022 the secretary of health suspended the certified nursing assistant license of Deanna Anita-Emily Leos (NC60087144). Leos failed to comply with a 2020 agreement that put her license on probation, required she inform her employer of the agreement, and provide quarterly performance evaluations.
