STATEWIDE (Nov. 9, 2021) — Today, the state of Colorado reactivated crisis standards of care for staffing of health care systems throughout the state. Crisis standards of care are guidelines for how the medical community should allocate scarce resources. In this case, the state is activating these guidelines specifically for staffing. Crisis standards of care for staffing of health care systems allows hospitals to implement staffing solutions to best meet the increasing medical needs of their communities. Upon activation, these crisis standards of care for staffing of health care systems may be implemented to best manage the current influx of patients who need care for COVID-19 or any other illness. 

The public health objectives of the Crisis Standards of Care for Health Care Staffing are to:

  • Expand the availability of health care workers and health care resources to combat the COVID-19 pandemic and to serve patients seeking non-COVID-19 related care;

  • Assure that guardrails and supports are in place to optimize workplace safety, health care worker resilience in the face of moral and physical stress, patient safety, and health outcomes of COVID-19 and non-COVID-19 patients.

“We want to be sure Coloradans know they can and should continue to access necessary health care. If you’re sick and need care, please go get it,” said Dr. Eric France, Chief Medical Officer. “Activating staffing crisis standards of care allows health care systems to maximize the care they can provide in their communities with the staff they have available.”

Health care workers have been at the forefront of the COVID-19 response for more than 20 months. Staff shortages due to COVID-19 illness, increased workloads due to hospitals working at capacity, and staff burnout are all making working conditions difficult and often outside the scope of conventional care. Health care systems must notify CDPHE when they activate and deactivate crisis standards of care for staffing.

The state is not at this time activating crisis standards of care for emergency medical services, crisis standards of care for hospital and acute care facilities, crisis standards of care for out of hospital care providers, crisis standards of care for specialty patient populations, or crisis standards of care for personal protective equipment. 

To help preserve hospital capacity, Governor Polis has issued Executive Order D 2021-135, which authorizes CDPHE to direct those facilities to transfer patients to prevent overwhelming the capacity of a facility and its staff. Hospitals can transfer patients that aren’t COVID-19 patients if that helps address capacity needs.

Hospitals are required (by federal law) to accept a transfer if they are able to provide the level of care needed and if they have capacity. Transfers are evaluated on a case-by-case basis to ensure Colorado has capacity and can provide the care required. A hospital must comply with the non-waived sections of the EMTALA (Emergency Medical Treatment and Labor Act), including the requirements to provide stabilizing treatment within the hospital’s capabilities and capacity prior to the initiation of a transfer to another hospital, and to provide a medical screening examination to any individual who comes to the emergency department and requests examination or treatment.

Will this reduce the level of care patients receive?
Upon activation, these crisis standards of care for staffing of health care systems may be implemented to best manage the current influx of patients who need care for COVID-19 or any other illness. It can affect the ratio of staff to patients, but allows hospitals to make these management decisions in times of crisis to manage and attend to more patients. 
 
Should I still go to the emergency room if I need care?
Yes. We encourage Colordans to continue to access any needed medical care. Staffing crisis of care standards do not impact a patient’s access to acute emergency treatment.
 
Does this mean there is a staff shortage at hospitals?

How will this help alleviate staff burnout? 

Hospital can take several steps to help staff, including:

  • Minimizing meetings and relieving administrative responsibilities during the defined crisis.

  • Reducing documentation requirements for health care workers.

  • Adjust staff schedules to minimize fatigue and promote resiliency.

 
Will I get care if I go to a hospital to for example, receive care for an unrelated COVID illness?
We encourage Colordans to continue to access any needed medical care.
 
Will this mean different things for different parts of the state? 
Hospital capacity varies across the state. In hospitals where there are more capacity limitations they may be more likely to implement these crisis standards of care for staffing. Health care systems must notify CDPHE when they activate and deactivate crisis standards of care for staffing.

Will people have to wait longer to receive care? 
Crisis standards of care for staffing of health care systems allows hospitals to implement staffing solutions to best meet the increasing medical needs of their communities. It will allow hospitals to attend to more patients. 
 
Will lesser trained staff take on more responsibilities?
Staff will receive cross-training, and healthcare workers can participate in “Just In Time” training to take over positions of skilled staff workloads. Training also can be provided to increase healthcare workers’ skill sets. 
 
Will more elective procedures be delayed? 
There has not been a statewide pause on restricting elective appointments and procedures outside of cosmetic procedures. Individual hospitals can pause those procedures if they choose to do so to reassign staff.
